Student application & information

616 Graham Ave.  Eau Claire, WI  54701

Dear Parent(s),

Thank you for your interest in the Children’s Dyslexia Center of Upper Wisconsin. The Scottish Rite Masons’ mission is to provide one-to-one Orton-Gillingham reading and spelling instruction to children with dyslexia between the ages of 6-18 at no cost to the families.  The center is unable to tutor children with severe hearing or vision deficiencies, autism, or behavioral or cognitive disabilities.  All children must be assessed before being admitted into the center.  The center does not provide this assessment.

Admission Process

Children will be considered for admission when all the information is completed and sent to the center director.  If you would like your child to be considered for admission into the program, please use the checklist below to be certain that you have included all required items:

  • Complete all questions on the application. Click Here:  Student Admission Application 2.28.2017
  • Send a copy of your child’s complete neuropsychological evaluation or equivalent testing that is not more than three years old.  The evaluation should include assessment in each of the following areas:
    • Cognitive ability
    • Reading
    • Spelling
  • Send copies of all medical and/or diagnostic evaluations completed on the child that may impact the child’s learning.
  • If the child is in a learning disabilities program, please submit copies of all diagnostic assessments and findings.

Notification Process

Once all the assessments are reviewed, parents will be notified of one of the following decisions:

  • The program is appropriate for the child, and the child may begin services.
  • More supporting material/testing is needed before a decision can be made.
  • The program is not appropriate for the child.
  • The program is appropriate and the child will be placed on the wait list.  The date of acceptance determines the wait list order.

Once Admitted Into the Program

  • The child receives two 50 minute tutoring sessions per week, falling on either Mondays and Wednesdays or Tuesdays and Thursdays for a fall session, spring session, and summer session by an Orton-Gillingham trained tutor.
  • Parents/legal guardians are required to accompany/supervise their child in and out of the center at all times. We offer a safe learning environment.
  • Parents are required to stay in the Learning Center while their child is in session, supervising all children under their care.
  • Parents need to plan for a one to three year commitment.
  • Parents or family members are highly encouraged to listen to the child read aloud a minimum of three times per week for 20 minutes.

I hope you find this information helpful. In order to help your child, attendance is important.  Please note that upon acceptance into our program, regular attendance is mandatory.  Multiple unexcused absences will result in your child’s immediate dismissal.

Please contact me if you have any other questions about our program or the application process.  The attached documents will provide you with more information about dyslexia and our services.
